List title catalog equivalence groups and their passes

/tnt/:tid/title_catalog/equivalents

For every title in the tenant's title catalog (or the search_index override), list each equivalence group (same title, region and offer type across variants) with its SKUs and the pass (NFT contract) the marketplace mints for each. These are the same groups used for equivalent_variants / equivalent_skus on entitlement add/status/list. With include_policy=true, also reads each playable object's _NFT_ACCESS list and reports the passes it's missing: a playable must admit every pass in every group with an offer that plays it.
